Position Summary:The Re-Entry Specialist position is responsible for case management responsibilities and ensuring member satisfaction by working collaboratively with cross-functional clinical and peer support teams to assess member needs evaluate transitional service options create transition plans coordinate care and monitor progress. For adults with recovery and re-entry issues recovery and success in life depends on good mental and physical health as well as having satisfying relationships safe housing purpose in life a sense of belonging and hope for the future. This includes assisting the members with acquiring and utilizing necessary skills to become self-sufficient and to not only effectively manage but also enjoy their lives.
QualificationsandRequirements
Meets the requirements of a Behavioral Health Technician
Must be 21 years or older.
High school diploma or GED required
CPR/FA certification required
Crisis de-escalation certification (i.e. Safety Care CPI etc.) required
Fingerprint Clearance Card
2 Step Tuberculosis Screen and Risk Assesment
PhysicalAttestation / Physician completedPhysical
Intermediate computer skills MS Office (Excel Word Outlook Teams) Internet Electronic Health Record etc).
Excellent communication organizational time management and critical thinking and problem-solving skills required.
May require driving and operation of a motor vehicle as needed
