MDS Nurse Job Opportunity at Trinity Village
Join Our Dedicated Team as a Full-Time MDS Nurse
Position: MDS Nurse
Location: Trinity Village
Type: Full-time (Monday through Friday)
Pay: Negotiable based on experience and education
About the Role
Trinity Village is seeking a compassionate and experienced MDS Nurse to help us deliver exceptional care to our residents. As an MDS Nurse you will work collaboratively to develop organize and monitor individualized care plans for residents from admission through discharge. Youll coordinate with an interdisciplinary team residents and families to ensure our care meets both state and federal standards.
Key Responsibilities
- Coordinate and manage Assessment Reference Dates (ARD) with the rehab director
- Complete accurate and timely Minimum Data Set (MDS) assessments per regulations
- Code MDS with data from medical record review observation and interviews
- Maintain MDS schedules and meet all required deadlines
- Conduct comprehensive resident assessments within 14 days of admission annually quarterly and as needed for significant changes
- Electronically transmitted MDS assessments to state databases and other entities
- Track record and analyze assessment defaults; implement corrective actions as necessary
- Assess for changes in resident conditions using reports and orders to update care plans
- Support and instruct staff on RAI Process Case-Mix PPS Medicare Medicaid and related systems
- Respond to residents and family concerns documenting and resolving issues
- Ensure compliance with all MDS Quality Measures Medicaid/Medicare RUGs and skilled care criteria guidelines
Qualifications
- Graduate of an accredited nursing program
- Current RN license to practice in North Carolina
- MDS certification preferred
- CPR certified or eligible for certification
- Experience in geriatric nursing supervisory roles and MDS documentation preferred
- Strong computer skills required
