Position Summary
The Social Worker supports the overall wellbeing of residents and their families by providing counseling advocacy and care planning. This position plays a key role in promoting a positive quality of life for residents at Sunshine Meadows through emotional support resource coordination and collaboration with the interdisciplinary team.
Key Responsibilities
- Provide social services to meet the emotional social and psychological needs of residents.
- Assist residents and families with adjustment to facility life including admission transition and discharge planning.
- Conduct assessments maintain required documentation and participate in care plan development.
- Serve as an advocate for residents ensuring their rights and preferences are respected.
- Provide counseling and support to residents and families during times of transition loss or crisis.
- Facilitate family conferences support groups and communication between residents families and staff.
- Coordinate community resources and referrals as needed (e.g. mental health services hospice community programs).
- Maintain compliance with all federal state and facility regulations regarding social services.
- Collaborate with nursing activities and other departments to support resident-centered care.
Qualifications
Qualifications
- Bachelors degree in Social Work (BSW) required.
- Current Kansas Social Work license (LSCSW or LMSW) in good standing.
- Previous experience in long-term care healthcare or geriatrics preferred.
- Strong communication advocacy and problem-solving skills.
- Ability to work effectively with residents families staff and community partners.
- Compassionate patient and committed to resident-centered care.
