About the role

Our Early Years Places are community hubs offering child health services supported playgroups family support parenting programs community engagement and early childhood education whilst focusing on early intervention and prevention to ultimately empower children and families. We are a supportive multidisciplinary team with a broad range of early childhood teachers and educators social workers midwives child health nurses and allied health qualified professionals committed to high quality work providing wrap around early intervention services.

Duties

As a Case Manager (Child & Family Practitioner) within our Early Years Centre you will

  • Manage a client caseload (families who have young children aged 0-8 years).
  • Work in partnership with families to assess strengths needs and risks develop implement and periodically review case plans.
  • Support the delivery of other programs such as playgroups and parenting programs.

Skills and Experience

We want you to be successful in your new role! This meansyoull have a degree in Social Work Human Services Psychology or related discipline and:

  • Demonstrated experience in working with young children and families including the application of high-quality practical intervention and casework skills.
  • High level understanding of preventative and early intervention approaches in child and family support; including demonstrated experience in partnering with parents in the development of case plans.
  • High level of interpersonal and written communication skills including the ability to communicate with families from a range of diverse backgrounds.
  • Proficient in working with First Nations and Culturally and Linguistically Diverse families.
  • Current drivers licence and valid Blue Card.
