- Develops and maintains professional relationships with staff physicians residents families community resources and third-party payers.
- Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process.
- Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal and updates POC with significant change and/or quarterly.
- Assess residents various health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.
Qualifications :
- Bachelor of Social Work (BSW)
- Experience in Long-Term Care or medical setting preferred
- Comfort and experience working with dementia populations a plus
- Flexibility to assist in our skilled assisted and residential living areas
