Breast Center Nurse Navigator (RN/LVN) Job Description
Department: Womens Center for Health Breast Center
Reports To: Breast Center Manager / Director of Womens Services
FLSA Status: Non-Exempt

Position Summary
The Breast Center Nurse Navigator is responsible for providing coordinated patient-centered support throughout the breast imaging and diagnostic process. This role focuses on timely follow-up in compliance with Mammography Quality Standards Act (MQSA) requirements assisting with breast procedures and guiding patients from abnormal findings through diagnosis and treatment initiation. The Nurse Navigator will serve as the primary point of contact for patients providers and referring physicians ensuring seamless communication and compassionate care.

Essential Duties and Responsibilities:

Patient Navigation & Care Coordination
-Receive and track all abnormal mammogram ultrasound and biopsy results ensuring follow-up is scheduled in compliance with MQSA timelines.
-Coordinate diagnostic imaging and biopsy appointments working closely with radiologists technologists and referring providers.
-Provide patient education regarding imaging findings procedures and next steps.
-Assist patients in overcoming barriers to care including transportation insurance authorization and scheduling issues.
-Facilitate communication between patients referring providers surgeons and oncology Support
-Assist with breast biopsies and other interventional procedures including patient preparation positioning and post-procedure care.
-Provide wound care instructions and follow-up calls after procedures.
-Maintain competency in infection prevention specimen handling and sterile technique.

Regulatory Compliance & Quality
-Maintain logs and documentation required for MQSA follow-up compliance.
-Participate in quality improvement initiatives and breast program accreditation activities.
-Monitor patient tracking systems to ensure timely completion of diagnostic work-ups.

Documentation & Communication
-Document patient encounters education and follow-up actions in the electronic medical record (EMR).
-Generate follow-up reminder letters and phone calls per MQSA and facility policy.
-Prepare reports for monthly quality meetings and annual MQSA reviews.

Qualifications
Required: Current Texas LVN and/or RN license in good standing.
Preferred: Two (2) years of nursing experience in womens health radiology oncology or related specialty.
Strong patient communication and education skills.
Familiarity with MQSA requirements breast imaging workflows and EMR systems (Epic preferred).
Ability to work independently and manage multiple priorities in a fast-paced outpatient setting.

Physical & Working Conditions
Outpatient clinical setting with direct patient contact.
Requires standing walking bending and lifting up to 25 lbs.
Potential exposure to body fluids and radiation (with appropriate protective measures).
