Organization Overview:The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30000 New Yorkers. We are two of New York Citys leading human services organizations meeting the needs of children youth and families homeless individuals and older adults through-out Manhattan and New York City. We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families. The Isaacs Center focuses primarily on the needs of children and low-income families out-of-school and out-of-work youth and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville and seek to deliver programs that are impactful innovative and intergenerational.


Intro/Program Description: Lincoln Square Neighborhood Center (LSNC) is a multigenerational program of Goddard Riverside that serves the educational recreational and cultural needs of the Lincoln Square community with a particular focus on residents of the Amsterdam Houses and Amsterdam Addition. LSNC Aging Services are funded by the New York City Department for the Aging (DFTA) and the New York State Office for the Aging (SOFA). More than 600 active members participate in the program which provides daily nutritious hot lunches along with a wide range of wellness educational artistic and social programming. Through comprehensive servicesincluding case management case assistance and health promotionLSNC Aging Services helps older adults (age 60) lead vibrant healthy and meaningful lives. The Naturally Occurring Retirement Community (NORC) program specifically supports residents age 60 of the Amsterdam Houses and Amsterdam Addition NYCHA developments. NORC services include social services health promotion activities and recreational cultural and social opportunities. Approximately 50% of participants are Spanish-speaking. The NORC operates in conjunction with the Older Adult Center which is open to all New Yorkers ages 60 and older.


Position Summary:Under the supervision of the LSNC Aging Services Program Director and the LSNC NORC Program Manager the Case Assistant provides essential support and services to older adult clients. Responsibilities include completing new client registrations and annual assessments updating client records and reassessing client needs on an annual basis. The Case Assistant assists with applications and recertifications for benefits makes referrals and connects clients to critical services such as post-discharge homecare legal assistance and medical care. The role also involves client advocacy with public and private systems collaboration with the on- site nurse and providing telephone reassurance and friendly visiting. Additionally the Case Assistant conducts home visits to ensure clients safety and well-being.


Schedule: Monday through Friday 8:00 am to 4:00 pm. Weekends: Cooling Center Administration if required (occasionally summer only rotation schedule)


Salary: $34130.58

Under the supervision of the Program Director and LSNC NORC Manager the Case Assistant provides direct client support and contributes to the effective operation of LSNC Aging Services. Responsibilities include:

Client Services

  • Provide advocacy information and referrals to connect clients with entitlement/benefits counseling health and mental health care food assistance housing resources caregiver referrals and other services in line with program guidelines.
  • Conduct new member registrations annual assessments re-registrations and health indicator requirements.
  • Perform home visits to homebound seniors to evaluate needs ensure safety link them to services and provide ongoing social contact.
  • Collaborate with the on-site nurse to coordinate medical care and assist with medication management.
  • Support clients with scheduling and coordinating appointments including providing reminders and follow-up.
  • Deliver emergency outreach and assistance as needed.
  • Provide friendly visiting and telephone reassurance to reduce isolation.

Documentation and Compliance

  • Maintain accurate timely client records in both written and electronic formats.
  • Enter data into client databases in accordance with agency and program standards.

Team and Program Support

  • Participate in staff meetings supervision sessions training programs and case conferences.
  • Contribute to program assessments evaluations and improvement plans.
  • Assist with the daily oversight of senior center activities and respond appropriately to emergencies.
  • Support operational needs including lunch ticket distribution and front desk coverage when required.
  • Assist with outreach engagement and promotion of program services within the community.

General

  • Perform other duties as required.

Qualifications and Educational Requirements

  • High school diploma required
  • Fluency in Spanish is required
  • Experience working with older adults preferred

Skills Knowledge and Abilities

  • Sensitivity to the strengths challenges and needs of older adults
  • Strong work ethic and reliability
  • Ability to multi-task and work effectively in a fast-paced environment
  • Strong written communication skills with the ability to write concise and clear case notes
  • Ability to communicate effectively with clients who may have physical disabilities sensory impairments (hearing vision) and/or mental health challenges
  • Ability to work collaboratively as a team member while also carrying out independent responsibilities

Computer Skills

  • Proficiency in Microsoft Office Suite (Word Excel Outlook)
  • Ability to use or learn electronic client databases
  • Ability to use or willingness to learn DFTAs PeerPlace/or new system (training provided)

Physical Requirements

  • Ability to circulate in the lunchroom for approximately one hour per day and greet clients in hallways escorting them to offices as needed
  • Preferred: ability to walk and stand frequently throughout the day
  • Ability to travel locally to conduct home visits accompany participants to appointments and follow up post-hospitalization as required

Work Environment

  • Senior center environment located within NYCHA housing facilities
  • Shared office space on the second floor accessible by elevator
  • Occasional fieldwork and home visits to support client needs
