Job Summary:

We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a per diem (PRN) position at a skilled nursing facility in Gloucester MA. This opportunity is ideal for nurses who are looking for flexible scheduling and enjoy working in a collaborative patient-focused environment.

Key Responsibilities:

Provide direct patient care in accordance with individualized care plans
Administer medications and perform treatments as prescribed by physicians
Monitor and document changes in residents conditions
Assist with admissions discharges and transfers
Maintain accurate and timely documentation in the electronic medical record (EMR) system
Work closely with the interdisciplinary team to achieve quality care outcomes
Promote a safe clean and supportive environment for residents

Required Requirements:

Active and valid LPN license in the state of Massachusetts
Current CPR/BLS certification
Minimum of 1 year of recent experience in a long-term care (LTC) or skilled nursing facility (SNF)
Strong clinical and communication skills
