Job Summary:
We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a per diem (PRN) position at a skilled nursing facility in Gloucester MA. Key Responsibilities:
- Provide direct patient care in accordance with individualized care plans
- Administer medications and perform treatments as prescribed by physicians
- Monitor and document changes in residents conditions
- Assist with admissions discharges and transfers
- Maintain accurate and timely documentation in the electronic medical record (EMR) system
- Work closely with the interdisciplinary team to achieve quality care outcomes
- Promote a safe clean and supportive environment for residents
Required Requirements:
- Active and valid LPN license in the state of Massachusetts
- Current CPR/BLS certification
- Minimum of 1 year of recent experience in a long-term care (LTC) or skilled nursing facility (SNF)
- Strong clinical and communication skills
