Full Time (80 Hours Biweekly)

Day Shift

Monday-Friday

Weekend rotation every 4th weekend

2 holidays per year

Sign on Bonus Eligible

General Summary

Provides comprehensive family-oriented nursing services to individuals in the home setting by use of the nursing process in accordance with physician orders the established plan of care corporate policy and procedures and the Nurse Practice Act.



Responsibilities

Duties and Responsibilities

Essential Functions:

  • Participates in the development of a written plan of care in accordance with the patients diagnosis patient and family needs physicians orders and the corporate policies and procedures.
  • Provides selected nursing procedures and treatments to patients as outlined in the plan of care and assigned by the primary nurse.
  • Notifies physician and other care agencies of changes in patients condition and care needs and obtains new orders as needed.
  • Assists with coordination of patient care through dialogue with other disciplines involved with the patient and documents same.
  • Participates in patient care conferences.
  • Documents observations and intervention in the patients clinical record in a timely legible and accurate manner.
  • Functions as a member of an interdisciplinary team encouraging and maintaining open communications at all times.
  • Fosters a climate of mutual respect and teamwork among co-workers and management.
  • Assist in the orientation of new staff.
  • Submits patient visit schedule to Scheduler and communicates significant changes to Primary Nurse.
  • Functions in a cost-effective manner in regard to utilization of time mileage supplies etc.
  • Meets or exceeds established productivity and quality assurance standards.
  • Seeks consultation guidance and direction from professional or administrative staff as needed.
  • Informs manager of perceived problem areas related to patient care corporate management or community relations.
  • Keeps clinical skills current with nursing practice norms.
  • Exhibits a comprehensive knowledge of commitment to and demonstrated ability in the practice of home health nursing.
  • Exercises sound judgment and works effectively under pressure.
  • Demonstrates a working knowledge of skills and services provided by each member of the multi-disciplinary team and the ability to relate these to identified patient needs.
  • Acquires and utilizes knowledge of community resources.
  • Elicits feedback from appropriate sources regarding the quality of services provided.
  • Demonstrates a professional attitude sound judgment and continuity of interactions with physicians other health care providers and community agencies.
  • Represents the VNA at community activities as delegated.
  • Promotes a positive corporate image.

  • Demonstrates a commitment to patient visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors as well as hazardous conditions; identifying opportunities to standardize processes and error proof systems that will lead to increased safety; and participating in safety education programs and root cause analysis as required.
  • Enhances professional growth and development through participation in educational programs current literature in-service meetings and workshops.

Qualifications

Qualifications

Minimum Education:

  • Certificate Program Post High School Vocational/Specialized Training Required

Work Experience:

  • 1 year Relevant experience. Required

Licenses:

  • Licensed Practical Nurse Upon Hire Required or
  • Licensed Practical Nurse Multi State License Upon Hire Required and
  • Basic Life Support Upon Hire Required or
  • Advanced Cardiac Life Support Upon Hire Required or
  • Ped Adv Life Support Upon Hire Required

Drivers License Statement:

Must possess a valid drivers license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.

Courses and Training:

  • Completion of accredited LPN program. Upon Hire Required and
  • Rhythm recognition course per unit of assignment. Upon Hire Required

Knowledge Skills and Abilities:

  • Knowledge of and commitment to the practice of nursing in the home.
  • Understanding of and commitment to a holistic approach to patient care with an understanding of family dynamics.
  • Demonstrated clinical interpersonal and communication skills.
  • Ability to demonstrate respect and appreciation for other professionals and paraprofessionals and to work cooperatively.
  • Ability to assess analyze and plan according to need utilizing the problem solving approach.
  • Willingness to request and accept consultation and direction.
  • Ability to maintain confidentiality of patient and organizational information.
