JOB SUMMARY

The Community Health Worker (CHW) will work with Community Based Organizations (CBOs) healthcare providers and departments/programs within the County to strengthen health equity serve as a liaison between community members and service providers address social determinants of health and provide education in culturally relevant and linguistically appropriate ways. One of the essential functions for this position is to network and establish and build relationships within communities we serve.

MINIMUM QUALIFICATIONS

Bachelors Degree from an accredited college or university in Community or Public Health Education Health Education Public or Health Administration or closely related field OR an equivalent combination of related education and experience working as a Community Health Worker Promotion Health Navigator Patient Advocate or similar.

A valid Drivers License accepted by the State of Utah is required at the time of hire. A valid Drivers License issued by the State of Utah must be obtained within six (6) months of hire to operate a County vehicle.

Due to the nature of this position the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

Experience working in specific zip codes/neighborhoods and with underrepresented communities strongly preferred.

Community Health Worker Certification is strongly preferred.

Ability to read write and speak in a language other than English strongly preferred

ESSENTIAL FUNCTIONS

  • Interacts with community members and connects them with needed resources.
  • Acts as a liaison between clinic staff patients community members and CBO partners.
  • Promotes services within the community by establishing relationships with local schools service providers community groups churches age-specific programs and others.
  • Develops culturally and linguistically appropriate material this may include translating.
  • Provides information education and outreach to individuals small groups and community partners and clinical staff.
  • Networks develops and maintains partnerships with community support agencies.
  • Provides advocacy to address health disparities at the individual and community levels.
  • Gathers and provides statistics on numbers of community members served and community health worker outcomes for Salt Lake County Health Department
  • Gathers feedback about additional community needs and gaps in services through focus groups surveys and individual discussion. Works with community agencies to help solve these gaps.
  • Schedule appointments with health/social services agencies for community members.
  • May provide information education and outreach on COVID-19 issues and teach community members ways to mitigate its spread in a manner that culturally supports the community being served.

  • Work effectively with people from a variety of social ethnic economic educational and professional backgrounds
  • Communicate and deal effectively with public government business and community leaders

Additional Information

Typical work hours are Monday through Friday 8:00 AM to 5:00 PM. This position offers a hybrid schedule; with at least two days in office and a max of three days working from home.

The position requires traveling to outreach community events and Community Based Organizations with some evening and weekend hours due to community events.
