Provides physical therapy treatment to all patients referred by a licensed physician. Is not able to perform initial evaluations. Works under the supervision of licensed Physical Therapists and the Clinical Therapy Manager.
treatment programs to assigned patients based on the treatment plan established by the physical therapist. Provides feedback to the physical therapist on a regular basis indicating the patients progress in treatment. Coordinates with other disciplines as needed.
responsibility for all documentation requirements in a timely manner including progress notes and discharge summaries.
group sessions as indicated by the Therapy Manager.
appropriate durable medical equipment (DME) in coordination with the proper team members.
in discharge planning and makes recommendations to the physical therapist.
appropriate rounds conferences inservices and meetings.
be assigned additional patients in the event that coverage is required as dictated by staffing needs.
patient attendance statistical records as required.
in supervision of PTA students on clinical affiliations.
other duties as assigned.
NYS PTA license and graduate of an Accredited PTA program. Associates degree required.
