RN Assessment Coordinator
Harlem Center for Nursing and Rehabilitationis looking for an RN AssessmentCoordinator to join the team at our Skilled Nursing Facility in Brooklyn NY!

RN Assessment Coordinator Responsibilities
Conduct and coordinatethe completion of MDS assessments
Maintain and updateall care plans and assessments
Examine and reviewresidents medical history
Inform other nursingstaff personnel of any persistent issues
Participate in theadmission process while assessing prospective residents nursing needs andappropriate placement
Coordinate casemanagement process
Ensure that thefacility adheres to all regulatory requirements and standards of practice inrelation to the MDS care planning process
Cooperate with inter-departmental personnel to ensure services can be adequately maintainedto meet the needs of the residents
RN Assessment Coordinator Requirements
Valid current RNlicense
Previous experience inlong-term care or skilled nursing facility
One-year job-related experience in the MDS process
Familiarity withvarious computer software
