Job Summary
A home-health hospice facility based out of Modesto CA is seeking an experienced Physical Therapist to join their team! If
Physical Therapist Salary: $125000 - $145000 annually
Physical Therapist Opportunity:
- Employment Type: Full-time permanent
- Schedule: Variable; seeing between 4-8 patients a day
- Setting: Home Health
- Assess and develop care plans: Conduct comprehensive evaluations of patients physical abilities and design individualized treatment plans in collaboration with the interdisciplinary care team
- Implement interventions: Provide therapeutic exercises and mobility training to maintain or improve function comfort and quality of life for patients with terminal illnesses
- Educate and support caregivers: Train family members or caregivers on proper body mechanics safe transfer techniques and ways to assist with daily activities to ensure patient comfort and safety at home
Physical Therapist Qualifications:
- A Doctor of Physical Therapy (DPT) degree from an accredited program.
- A license to practice Physical Therapy in California
- At least 1 year of Physical therapy experience required
- Experience in home health setting is a plus
