NI-Advanced Practice Provider Family Medicine, Same Day Care

Position Highlights:
UW Health in northern Illinoisseeks a passionate Advanced Practice Provider(APRN or PA-C)to join our Same Day Care initiative atUW Health in northern Illinois.

With a focus on providing same-day access to patients within our system the Same Day Care provider is responsible for seeing patients at clinics throughout the region. Appointments include acute issues chronic issues mental health assessments medication rechecks and physicals (school sports employment). We are looking for APPs with excellent communication skills both in educating patients and family members and in communicating care plans to a multi-disciplinary team.

Your clinical support includes the following:

• Flexible schedule working Monday through Friday with no weekend requirement

• Team-based support from providers in your outpatient clinic

• Telemedicine opportunities

• Robust onboarding program and mentorship

Qualifications:

• Masters degree from accredited PA program or completion of a masters level advanced practice nurse program.

• Active APRN or PA license in State of IL and active DEA license or ability to apply for such license prior to or upon hire.
