Job Description
Job Purpose
We believe that the childs support system is the most valuable blessing. Therefore we seek to provide physical emotional and spiritual support to the families and the children we serve. Essential to such support is the appropriate and positive interaction between the childs caregivers and the staff.
Essential Duties and Responsibilities

Reviews updates and facilitates timely turn-around of signed provider orders plans of care and other documentation to and from home health and DME companies to ensure continuation of care. | 70% |
Collaborates and consults with the multi-disciplinary team including but not limited to physicians nurse practitioners nursing social services and insurance companies along with patients and families to ensure safe and effective coordination of care. | 10% |
Assist nursing staff to submit referrals and coordinate procedures with external clinics and facilities by obtaining authorization and following-up with families and providers. | 5% |
Provide direct patient care when needed. | 5% |
Documents all communication with patient/ family medical provider insurance company and community partners in the electronic health record. | 10% |
Qualifications
Education: | Associates or Bachelors Degree |
Experience: | 1-2 years of case management experience preferred; 2-3 years of clinical experience with medically complex patients preferred. |
License/Certification/Credential: | RN |
