What Youll Do:
Provide in-home or on-site care at private residences senior living communities and long-term care settings
Manage chronic conditions preventive care and mental health needs for an aging population
Collaborate with interdisciplinary teams to deliver whole-person senior-focused care
Build trusted relationships through empathy consistency and patient education
Support care access and equity by removing barriers for vulnerable seniors
