Responsibilities:
Assess develop implement and revise clients individualized plan of care for ordered Physical Therapy services.
Treat clients to relieve pain develop and restore function and maintain maximum performance using appropriate physical therapy treatments modalities and procedures as stated in the physical therapy plan of care.
Observe record and report the clients reaction to treatment and any changes in the clients condition to the physician and RN Case Manager.
Educate the client/caregiver and health care team in the phases of physical therapy in which they may assist the client in the use of prosthetic and orthotic devices and durable medical equipment as appropriate.
Develop prepare and maintain individualized client care progress records with accuracy timeliness and according to agency policy and procedures.
Participate in interdisciplinary meetings to ensure optimal and comprehensive care for all clients served.
Qualifications:
Must have active Licensure for Physical Therapy in the state of MN.
Must have Drivers License auto insurance and reliable vehicle.
Willing to travel to patients home to perform evaluations/visits.
Benefits & Perks:
Competitive Pay! $42-$53.00/hr mileage reimbursement. First 45 days paid hourly then change to paid per visit.
Individual compensation packages are based on various factors unique to each candidate including skill set experience qualifications and other job-related reasons.
