Job Summary
INTERNAL/EXTERNAL POSTING
The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest Kettle & Stony Point Watford and Sarnia. Our interdisciplinary team provides primary health care health promotion and community development services.
The North Lambton Community Health Centre is currently seeking applicants for the following position:
1.0 FTE NURSE PRACTITIONER
37.5 hours/week fulltime with benefits
The Nurse Practitioner (RN-EC) delivers comprehensive primary care and contributes to the planning implementation monitoring and evaluation of programs and services for individuals families and the community. As a key member of the interdisciplinary team the Nurse Practitioner works both independentlyproviding the full scope of primary and outreach careand collaboratively by consulting with other clinical team members as appropriate.
Qualifications:
- Undergraduate degree in nursing from a recognized university (or equivalent combination of education and experience).
- Registration in the Extended Class with the College of Nurses of Ontario.
- Three to five years nursing experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting an asset.
- Experience in program development implementation monitoring and evaluation.
- Proficiency in the use of computers and various software applications.
- Participates in CNO Quality Assurance Program.
- Has and maintains CNO Professional Liability Insurance.
This is a fulltime position working 37.5 hours per week Monday to Friday. This position will be based at our West Lambton Site with allocated time at outreach sites. Experience will be taken into consideration on our exciting new 6-step wage grid. Vacation benefits medical benefits the Healthcare of Ontario Pension Plan and paid vacation are included.
