Must have acute care adult hospital case management Unit Information: SOCIAL WORKER (917)Facility: HCA FLORIDA LAWNWOOD HOSPITALAddress: 1700 S 23rd Street Fort Pierce FL 34954Floor:Building:Cost Center: 917General InformationTell us about the unit(s): Number of Beds: 344 beds at acute care hospital (excluding BH and acute rehab) Patient Ratios: SW if pair with Matrix of another RN will work Complex case of 8 or 9. If they are just covering DC planning then will have 1:25 or up to 30 Documentation System: MIDAS/Meditech Nate Navihealth Care Team Assignment Call Requirements: All work every third weekend Floating policy: All Social workers float-except trauma.
