Job Summary:

The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are critical.
Location:Lubbock Texas United States
Responsibilities:

- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management services.
- Provide hospice care.
Required Skills & Certifications:

- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
- PICU experience
Preferred Skills & Certifications:

- Not specified
Special Considerations:

- Position may float between units.
- Weekends are required variable.
- Block schedules are required.
- PICU experience is required.
- First-time travelers are accepted.
Scheduling:
- 1-year contract
- Ratios: 1:2/3
