Job Description

SH-722 Project Manager Indianapolis IN.

Summary:

Position Summary:

Under the direction of the Government Affairs Director the Project Manager (PM) will assist in the statewide implementation and coordination of EPSDT (Early and Periodic Screening Diagnostic and Treatment) and Targeted Case Management (TCM) services for Medicaid-eligible youth transitioning into and out of carceral or juvenile justice settings. This role is critical for ensuring compliance with federal regulations including Section 5121 of the Consolidated Appropriations Act (CAA) of 2023 and aligning service delivery with Indiana Medicaids whole-person community-based care goals.

Key Responsibilities

Strategic Program Oversight

  • Coordinate the development and implementation of statewide policy and operational models for EPSDT and TCM service delivery to juvenile justice-involved youth.
  • Facilitate internal collaboration to design and oversee a Medicaid delivery model that ensures continuity of care 30 days before and after discharge from carceral settings.
  • Ensure effective collaboration with the Indiana Department of Correction (IDOC) Department of Child Services (DCS) courts healthcare providers and Managed Care Entities (MCEs).

Policy & Compliance

  • Collaborate on Medicaid policy to support the implementation of Section 5121 of the CAA of 2023.
  • Assist the state with 1115 waiver preparation and other Centers for Medicare & Medicaid Services (CMS) deliverables as needed.
  • Ensure all program activities comply with CMS EPSDT and TCM requirements and maintain an audit-ready status.

Care Coordination & Systems Integration

  • Develop integrated workflows that connect juvenile justice facilities managed care organizations and behavioral health and primary care services.
  • Establish clear mechanisms for referral enrollment and follow-up care during the pre- and post-release periods.
  • Ensure the implementation of trauma-informed and culturally competent care planning models.

Data Quality & Evaluation

  • Oversee the development of key performance indicators (KPIs) dashboards and program evaluation strategies.
  • Monitor metrics related to service access care continuity and health outcomes for the target population.
  • Work within the Family and Social Services Administration (FSSA) to ensure the integration of eligibility claims and transition data.

Stakeholder Engagement & Technical Assistance

  • Facilitate statewide stakeholder groups task forces and training sessions for cross-sector teams.
  • Provide technical assistance to MCEs juvenile justice staff and community partners.
  • Assist the Government Affairs Director as needed with CMS discussions and presentations.

Qualifications:

  • Required: Bachelors degree in public health public administration social work healthcare administration or a related field. A Masters degree is preferred.
  • Required: Proven experience (3 years) in project management preferably within Medicaid behavioral health justice-involved populations or a related public sector field.
  • Required: Demonstrated knowledge of Medicaid programs specifically EPSDT and Targeted Case Management services and regulations.
  • Preferred: Familiarity with Section 5121 of the CAA of 2023 and Indianas Medicaid landscape.
  • Required: Strong ability to collaborate with diverse stakeholders including state agencies managed care organizations and community providers.
  • Excellent verbal and written communication skills with proficiency in Microsoft Office Suite (Word Excel PowerPoint Teams).
  • Must be a quick learner with the ability to adapt to frequent process changes and manage multiple priorities effectively.
