Job Summary

Bachelors degree in psychology social work or related field.
* Two years experience in a behavioral health care setting.
* Developmental Pediatrics experience preferred.
* Knowledge and understanding of behavioral health diagnoses critical thinking skills and psychosocial assessment skills.
* Knowledge of hospital and community resources.
* Computer knowledge and ability to effectively utilize and document patient information in the electronic medical record.
* Knowledge of medical/behavioral health terminology.
* Demonstrated customer service and organizational skills.
* Effective listening skills.
* Ability to show empathy.
* Understand cultural norms related to mental health issues.
* Knowledge of psychiatric medications.
* Knowledge of insurance requirements.
* Ability to interact with various cultures served by Childrens in a culturally competent manner.

The posted salary represents a market competitive range based on salary survey benchmark data for similar roles in the local or national market. When determining individual pay rates we carefully consider a wide range of factors including but not limited to market indicators for the specific role the skills education training credentials and experience of the candidate internal equity and organizational needs.

In addition to your salary this position may be eligible for medical dental vision retirement and other fringe benefits. Positions that require night weekend or on-call work may be eligible for shift differentials or premium pay.

Position Summary

The Clinic Case Coordinator works closely with the clinicians to assist patients with their non-medical needs related to admission and discharge planning and coordination of outpatient treatment. The role would interface with parents referring clinicians and community partners to concordant communication to and from the clinic clinical team. The role would assist with issues related to admission and discharge planning and communicate with other provider groups for follow up or step down care. The role reports to the department manager and works closely with the clinical team. The role works both independently and in conjunction with the clinician team to address complex patient needs in the outpatient clinic and inpatient unit.

Responsibilities:

  • Referral management
    • Manages patient phone calls and resolves their concerns and communicates with/refers as appropriate. Potential phone calls with referring parties as well
  • Completion of prescreen (Intake) form to determine level of care
    • Educates patients and their families to help them understand what services are available and how these would benefit their childs unique developmental and/or behavioral needs
  • Scheduling Intake with appropriate therapist and/or bridging provider
    • Assesses patients needs and uses critical thinking and problem-solving skills to help patients and their families access resources at Childrens or in the community
    • Triage with clinical teams as needed to determine appropriate level of care
  • Resource navigation for external and/or internal services
  • Potential wait list management

This role will be primarily based in St. Paul and will support our partial hospitalization in Roseville as well.
