Job Description
Saint Lukes Hospital in Kansas City has an opportunity for a Heart Failure Educator. THis position provides individualized heart failure education to patients admitted with diagnosis of heart failure and their appropriate care giver(s) as a supplement to the education provided by the bedside
nurse and interdisciplinary team.
Hours: 8 am -4:30 pm 2 days per week. No weekends.
Emphasize the importance of self-management and monitoring through education provided.
Assist with care coordination of inpatients by providing feedback to the treatment team RN and care progression team of any obstacles to successful transition and follow-up identified during education session.
Utilize available resources tools and referrals for patients who are identified to possibly benefit from additional supports after discharge.
Assist program manager with planning and executing support group meetings.
Job Requirements
Applicable Experience:
1 year
Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)
Bachelors Degree - Nursing
