The SEWB Care Coordination Worker is responsible for ensuring timely and accurate assessment of client needs followed by effective linkage to appropriate internal and external social and emotional wellbeing services that support positive health and wellbeing outcomes.
Essential Criteria
Someone reliable who shows up and brings a high level of skill to their work.
Be willing to learn all aspects of the job
Able to take direction and work well with others.
Flexible and ready to adapt to the changing needs of the service and community.
Can stay calm and manage stressful situations effectively.
A people person whos comfortable working independently when needed.
Able to communicate respectfully and sensitively with Aboriginal and Torres Strait Islander people and committed to cultural safety.
Has a good understanding of Aboriginal culture and history.
Willing to contribute to improving how we do things.
Organised and able to manage time well even when juggling multiple tasks.
Great interpersonal and communication skills.
Holds a current and unrestricted WA Drivers Licence.
Current Working with Childrens Chek or ability to obtain upon appointment
National Police Clearance
Uphold the quality ethics and professionalism required to be an effective team member
