Position Overview
We are seeking a dedicated and experienced Population Health Nurse to join our team. The ideal candidate will play a crucial role in assessing community health needs developing and implementing health promotion initiatives and collaborating with interdisciplinary teams to enhance health outcomes.
Key Responsibilities
- Transitional Care Management (TCM)
- provides telephonic outreach to all patients that have been discharged from a hospital
- Schedules TCM visit with PCP
- Quality measures (mostly PCP for insurance companies) possible in the future for specialist (Cardio Plum and GI)
- Collaborating with providers and insurance companies to improve quality metrics and close care gaps including wellness exams cancer screenings and chronic disease monitoring
- Navigate insurance portals to support quality reporting
- Triage urgent patient calls and escalate care needs
- Task buckets
- Calling patients regarding diagnostic results/ lab results
- Refill requests
- Answering messages from the patient portal
- I Care (VFC vaccines) tracking we are staying on top of the requirements and calibrating thermometers when needed
Qualifications
Current active and unrestricted Registered Nurse (RN) license in the state of Illinois.
Bachelor of Science in Nursing (BSN) required; Masters degree in Nursing Public Health or related field preferred.
- Current BLS certification
Minimum of 2 years of nursing experience with at least 1 year in community or public health settings.
Certification in Population Health Nursing or Public Health Nursing is highly desirable.
Strong understanding of social determinants of health and health equity principles.
Excellent communication organizational and analytical skills.
Proficiency in electronic health records (EHR) and data analysis tools.
