Youll provide support guidance and resources helping patients get the care they need while connecting them to the community services that make life easier.
What Youll Do:
Offer crisis support counseling and resource referrals for patients and families
Help plan safe discharges and transitions of care
Coordinate with healthcare teams to keep patients supported and informed
Connect patients to community resources for housing food transportation and more

Job Requirements
Education: Masters Degree (MSW) from a school of social work accredited by the Council of Social Work Education REQUIRED
