Location: The Village of Meyerland an Assisted Living with Memory Care senior community.
Administer medications safely and accurately per physician orders and regulatory standards
Oversee medication aides and conduct monthly medication reconciliation and audits
Coordinate daily staff assignments to ensure consistent resident care
Communicate effectively with physicians families and care teams regarding resident needs
Provide direct care including ADLs: bathing dressing grooming toileting mobility and incontinence support
Respond to medical and behavioral emergencies with sound clinical judgment
Complete clinical documentation including assessments evaluations and service plans
Document incidents and communicate promptly with leadership
Monitor care team documentation for accuracy and completeness
Support orientation and ongoing training of care staff
Assist with scheduling in collaboration with leadership
Attend required in-services and continuing education
Maintain professional appearance and hygiene-
Promote respectful and effective communication across teams and families
Perform other duties assigned to support resident care and operational excellence
Minimum Requirements:
Current and valid LVN/LPN license in the state of employment
Basic Life Support (BLS) / CPR certification
Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care
Proficient in Electronic Health Records (EHR) and basic computer skills
Strong verbal and written communication skills
Demonstrated critical thinking and problem-solving abilities
Knowledge of HIPAA regulations and compliance standards
Ability to respond with patience and understanding to dementia-related behaviors
Physical ability to perform essential job functions with or without reasonable accommodation
