Position Overview:
The Care Coordinator is responsible for supporting clients by assessing barriers and recommending resources and solutions relevant to each client. This position will work with clients and their families and in partnership with mental health providers primary care providers educators and other community providers to help clients achieve their health care goals.

High School diploma or equivalent required with one year of relevant work experience health care mental health customer service administrative experience.

Position Details:
Location: Fraser Bloomington/Virtual Office & Community-Based
Schedule: Monday-Friday between the hours of 8 AM - 6 PM (full-time)
Pay: The starting pay range for this role is $22.60 - $23.00 an hour depending on qualifications.
