_{We are seeking a Certified Hand Therapist (CHT) to join our rehabilitation team in Grand Junction CO. This role involves providing expert-level therapy to patients of all ages with a focus on upper extremity rehabilitation. The therapist will assess plan and implement individualized care plans to promote optimal functional recovery monitor progress through outcome measures and produce accurate timely clinical documentation.}

_{Provide comprehensive evaluations develop care plans and deliver specialized hand therapy services to patients of all ages.}
_{Modify treatment approaches based on patient progress and goals to ensure optimal recovery outcomes.}
_{Educate patients and caregivers on exercises home programs and techniques to support long-term success.}
_{Produce accurate timely documentation that meets all regulatory and professional guidelines.}
_{Utilize data from outcome measures to evaluate intervention effectiveness.}
_{Communicate clearly and effectively with physicians staff and leadership regarding patient care and departmental needs.}
_{Demonstrate an understanding of cost-effective care including billing coding and resource management.}
_{Support quality and performance improvement initiatives within the rehabilitation department.}

_{Qualifications}

_{Required Education & Credentials:}

_{Bachelors degree or higher in Occupational Therapy or Physical Therapy from an accredited program.}
_{Current Basic Life Support (BLS) certification.}
_{Unrestricted license to practice as an Occupational Therapist or Physical Therapist in the State of Colorado.}
_{Certified Hand Therapist (CHT) credential from the Hand Therapy Certification Commission (HTCC) with ongoing recertification every 5 years.}

_{Preferred Background}

_{Previous experience in upper extremity/hand therapy in a clinical or hospital setting.}
_{Strong interpersonal organizational and documentation skills.}
_{Ability to work collaboratively in a multidisciplinary environment.}
