Job Summary
Position: Registered Nurse - Outpatient
Start Date: 10/6/2025
Duration: 13 weeks
Location: Mattoon IL 61938
Shift Type/Shift Times/Hours/Day: Days 8:00 AM - 4:30 PM 8
Shift Notes: M-F 8 am - 4:30 pm (or until assigned patient visits are complete) / 40 Hour Guarantee
Weekend Requirements: Every 4th Weekend
On-Call Requirements: Yes call required

Travel Policy: Applicants within 50 miles of the facility are not eligible. Applicants who were direct employees within the last 2 years are not eligible.
Territory: 50-60-mile radius.
Caseload: max 12
Mileage: Reimbursed at the standard IRS rate; however there is a large possibility that they would be assigned a fleet vehicle
EMR: Netsmart: MyUnity
Dress Code: Royal Blue Scrubs
Further Specifics:
- Nonbillable Hours: 16 hours
- OT/Holiday Markups: 1.25x Regular Rate
- Call Back Markup: 1.3x Regular Rate
Registered Nurse is responsible for identifying patient/family needs and for providing nursing and supportive care for terminally ill patients and their families/caregivers. Manages a caseload of patients/caregivers as assigned.
Submission Requirements- 2 years of Experience - Required
- IL RN License - Required
- BLS (AHA) - Required
- Hospice Experience - Required
- Dedicated Hospice Admission Experience - Highly Preferred
