About Facility:
Home Health
Radius rule: Travelers permanent address must be 50 miles outside of home unit
Department Information:
o 6 visits a day
o There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
o Minimum Years of Experience required 1 year Home Health
o Will you accept a first-time traveler Yes if seasoned to home health
Patient Types/Common Diagnoses:
o Chronic diseases; Orthopedics/Ortho Surgery
Required License National Certification Certs (BLS ACLS etc.) or other position specific requirements such as Drivers License and/or Insurance:
o Drivers License and proof of Insurance
o PT: BLS; NC State License
Skills required:

Discharges
Supervision of PTA
Support within the Department:
o Departmental leadership

o EPIC Rover view only
o Focura - will upload discharge instructions and other important docs such as wound pictures
o Samsung note 9 phones
o HCHB Homecare Homebase
Orientation (facility and unit):
o NEO
o 5 days orientation
Scheduling:
o Weekend rotation none
o No call
o No major holidays
o RTO: Manager approval
o Schedule cycle: every 13 weeks
