Care Navigation RN Facilitator
Hospice of Marion County A Member of Empath Health
Schedule: Monday Friday 8:00 a.m. 5:00 p.m.
Hospice of Marion County part of the Empath Health family is seeking a Care Navigation RN Facilitator to join our dedicated team. This role provides leadership and coordination for the Care Navigation team ensuring excellent patient transitions collaboration with community partners and seamless access to Empath Healths Integrated Network of Care.
Key Responsibilities
Lead support and evaluate the Care Navigation team to ensure effective daily operations and outstanding patient service.
Collaborate with out-of-area partners to facilitate smooth patient transfers and appropriate care coordination.
Serve as a key resource and advocate for patients families and care partners throughout the admission and transition process.
Partner with internal departmentsincluding Finance Reimbursement and Interdisciplinary Teamsto ensure continuity of care.
Maintain compliance with hospice regulations and documentation standards within the EMR.
Participate in ongoing review and improvement of Care Navigation processes promoting Empath Healths mission and values.
Qualifications
Associates degree (A.A.) or equivalent required; Bachelors degree preferred.
Current Florida RN license required.
Minimum two (2) years of hospice experience strongly preferred.
Valid Florida drivers license and reliable transportation required.
Strong communication leadership problem-solving and organizational skills.
Proficiency in Microsoft Office and Electronic Medical Record (EMR) systems.
