DISCLOSURE - MULTI-EMPLOYER ROLE:


This W-2 hourly Direct Support Professional position includes responsibilities that span across the following two affiliated organizations all under shared leadership:


FOCUS #1Community Wellness Psychiatric Rehabilitation Program

FOCUS #2 HolBrock Estates at 1638 Ruxton Avenue LLC


You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies with hours responsibilities and compliance obligations tracked separately for each.


This role is structured as an up to 32-hour per week position with approximately 16 of the total weekly hours allocated to Community Wellness Psychiatric Rehabilitation Program and 16 of the total weekly hours allocated to HolBrock Estates at 1638 Ruxton Avenue LLC. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care each employer will manage its own onboarding HR and compliance processes in accordance with applicable laws and organizational policies.

Client Service Location Expectations:

This position is based on-site at the supportive housing program located at Holbrock Estates at 1638 Ruxton Avenue Baltimore MD 21216. All services are to be delivered in person at this location. The staff member is expected to be present on-site during scheduled shifts provide support within the residence and ensure documentation and responsibilities are completed in alignment with program protocols. Remote work or off-site service delivery is not applicable for this role.


Caseload Makeup:

The assigned caseload for this position consists of eight (8) Category A clients who reside in the community and may be serviced at 3310 Eastern Avenue Baltimore MD throughout the community or in the community meeting room of 1638 Ruxton Avenue location.

DIVISION: Operations


CLASSIFICATION:

This is a full-time W-2 employee position totaling 32 hours per week with hours allocated across three affiliated organizations as follows:

  • HolBrock Estates at 1638 Ruxton Avenue LLC:
    Part-time W-2 employee approximately 16 hours per week (50% of total weekly hours)
  • Community Wellness Psychiatric Rehabilitation Program (CWPRP):
    Part-time W-2 employee approximately 16 hours per week (50% of total weekly hours)

While each assignment is classified as part-time under its respective entity the combined total constitutes a full-time 32-hour per week role across the organization network. This structure ensures shared staffing efficiency and cross-program collaboration in alignment with operational and regulatory needs.


WORK SCHEDULE: Every Sunday to Wednesday from 12:00 AM to 9:00 AM each day.

COMPENSATION RANGE:
Compensation for this position ranges from $15.00 to $18.00 per hour and is commensurate with experience expertise verified credentials and available company budget.

ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel.

GENERAL DUTIES AND RESPONSIBILITIES:

  • Assist supervisor with work-related requests.
  • Collaborate with and serve as program POC for external treatment providers and outside agencies as needed.
  • Responsible for following regulations of COMAR CARF any other regulatory body and company policies/ procedures related to service delivery and documentation and co-facilitating orientation of all new hires to these as well.
  • Support the maintenance of a safe environment by participating in training and drills as requested.
  • Maintain confidentiality of records relating to clients treatment.
  • Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by the supervisor.
  • Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.

PHYSICAL DEMANDS:

  • Frequent lifting bending walking and standing
  • Ability to lift up to 40 lbs and assist with resident transfers
  • Use of PPE and participation in resident lifting or repositioning


WORKING CONDITIONS:

  • Licensed assisted living home environment
  • Close interaction with residents experiencing physical or cognitive decline
  • High-contact hands-on caregiving role with regulated standards and documentation

The Psychiatric Rehabilitation Program (PRP) is a core service of Community Wellness Psychiatric Rehabilitation Program designed to support individuals living with serious mental illness in achieving stability independence and improved quality of life. Through individualized service planning case management health home coordination and peer support the PRP helps clients navigate housing employment education and health care access. The Direct Support Professional (DSP) in the Psychiatric Rehabilitation Program (PRP) is responsible for supporting individuals with serious mental illness and/or co-occurring disorders in achieving greater independence and community integration. SUMMARY OF POSITION RESPONSIBILITIES:

The Direct Care Staff at a Licensed Assisted Living Facility provides essential daily care medication support and supervision to residents in accordance with the Maryland Department of Healths COMAR 10.07.14 regulations. This role combines the responsibilities of a Certified Medication Technician (CMT) personal care aide and household support staff working under the direction of the Operations Manager 5 during the day shift and the Operations Supervisor 5 during the night shift.
