Acts as a teammember throughout the consortium on a wide variety of nutritional needs. Thisposition serves as part of the multidisciplinary healthcare team to providepatient care that is integrated compatible with the patient-focused medicalgoals and culturally appropriate in accordance with SEARHC s strategic goals. Position Complexities Required to make daily decisions on nutritional management about awide range of clinical problems for both inpatients and outpatients. This necessitates keeping up to date in allclinical areas to manage daily problems as well as on call. Complex problems may require literaturesearches consultation with medical staff other dietitians as well as otherhealth professionals to determine a management plan. They must prioritize their time within theconstraints of food service short term admissions and coordination ofnutritional care with other members of the health care team. Responsible for maintaining and applyingcurrent scientific knowledge about food and nutrition to individuals and groupsin states of health and disease to promote optimal health outcomes within thesocial economic and cultural context of the Alaska Native/American Indianpopulation in accordance with SEARHC s primary objectives and the policies andprocedures of the medical nutrition services department. Must translate complex regulations from theJoint Commission (TJC) and state/federal standards into everyday practice. Provides counseling for patients and/or othercaregivers on nutritional principles dietary plans and food selections takinginto account age-specific cultural and ethnic requests. Participates in physician hospitalnutrition services and health care interdisciplinary teams and serves as theconsultant on nutritional care. Accountabilities Ambulatory Medical Nutrition Therapy Serves on and works closely with themultidisciplinary ambulatory healthcare team to provide individual or groupnutrition screening assessment diagnosis education intervention monitoringand evaluation using the Nutrition Care Process for ambulatory nutrition care provided using established departmental protocol inpatient s medical record including relevant coding. Inpatient Medical Nutrition Therapy Conductnutrition screening assessment diagnosis education intervention monitoringand evaluation using the Nutrition Care Process for all hospitalized patientsat all levels of care and age. Document nutrition care provided usingestablished departmental protocol in patient s medical record. Long Term Care Works closely with the multidisciplinarylong-term care team to provide nutrition guidance as well as individualassessments diagnosis education intervention monitoring and evaluationusing the Nutrition Care Process for residents. Document nutrition careprovided using established protocols in patient s resident s record. Hospital Nutrition Services Work closely with the Hospital NutritionServices staff to clarify or communicate patient diet orders foodpreferences menu choices and formula needs. Complete menu revisionsmodifications and nutrient analysis of inpatient menus. Team Membership Serves as a member of the Diabetes Teamand Patient Care Conference when patient workload permits. Provides departmentleave and on-call coverage in collaboration with the Supervising Dietitian. Assistswith teaching the diabetes prevention program be involved with the communityevents and facilities and other activities that promote good nutrition. Provides weight loss assessment and bariatricsurgery assessments and follow up in SEARHC s program and supports its patientsthroughout the consortium. Provides nutritional education orinformation via community health fairs and other outreach arenas when requestedand/or needed. Other duties as assigned by the Supervising Dietitian and/orDivision Director or VP Baseline Qualifications Education/Experience Bachelor s Degree from school whose Nutrition DieteticInstitutional Management Programs is accredited by Commission on Accreditationfor Dietetics Education (CADE) of the American Dietetic Association (ADA) andcompletion of an approved dietetic internship program. Two years experience as a clinicaldietitian is preferred. License/Certification Dietetic Registration by the Commission on Dietetic Registrationand licensure from the State of Alaska Department of Commerce Community andEconomic Development Division of Corporations Business and ProfessionalLicensing required. Certified Diabetes Educator (CDE) Desirable KnowledgeSkills & Abilities: Knowledgeof Advanced level of medical nutritiontherapy and clinical knowledge. Working level of computer knowledge. Some knowledge of human behavior andtechniques for effective behavior change. Skillsin Clinical nutrition services: screeningassessment diagnosis education intervention monitoring and evaluation usingthe Nutrition Care Process for patients at all levels of care and age. Skills in education counseling andteaching. Competent using basic computer software:Microsoft Word Publisher PowerPoint Resource Patient Management System(RPMS) or other electronic medical record charting software.
