A 64-bed adult psychiatric facility Lasting Hope Recovery Center is a place of healing. All of the rooms are private. At Lasting Hope Recovery Center patients find comfort and a team of caring professionals working for them. Often mental illness and substance abuse occur at the same time. CHI Health Lasting Hope Recovery Center offers an entire range of treatment and prevention services to meet the needs of patients experiencing both challenges. Treatment is built on personal strengths and support systems that treat individuals experiencing these illnesses.

Looking for a flexible PRN role with a big impact Join our team as a Care Coordination Social Worker!
Why this role
- Flexible Schedule:Monday-Friday shifts (7-8 hours) with adaptable start times (7 AM - 8:30 AM). Only one weekend rotation every 8 weeks!
- Diverse Experience:Work across three metro inpatient behavioral health campuses with optional remote work for our Kearney location.
- Meaningful Work:Support diverse patients strengthen your skills in care coordination and discharge planning and collaborate with interdisciplinary teams.

What youll do:Youll be a key part of our care team focusing on patient advocacy crisis intervention and ensuring safe transitions and continuity of care. This includes:
- Planning safe discharge and continuity of care for all patient populations.
- Educating patients and families on self-care and health management.
- Providing psychosocial support crisis intervention and referrals.
- Coordinating community resources and managing transitions of care.
- Identifying and addressing social determinants of health (e.g. housing food insecurity).
- Collaborating with teams for patient/family care conferences.
- Ensuring compliance with regulatory guidelines and supporting performance goals.

Job Requirements
Education: Bachelors or Masters Degree (BSW or MSW) from a school of social work accredited by the Council of Social Work Education.
License/Certification: Social Work Certification in the State of NE and/or IA
