About the Role
The Medical Team Lead Pharmacist provides expert clinical pharmacy services supporting safe effective and evidence-based medication management across the inpatient medical wards. The role involves supervising and mentoring pharmacists leading quality improvement initiatives and working closely with the multidisciplinary team to optimise patient outcomes. They also contribute to policy development education and service planning within the pharmacy department.
Classifications for this position will range from SX6-SX81 ($108820.40 - $123905.60 per annum).

POSITION PURPOSE
The Clinical Pharmacist Medical grade 3 is a clinically focused leadership role. They will provide acomprehensive and advanced clinical pharmacy service to St Vincents Hospital Medical units. The focuswill be on the General medicine units but also includes renal medicine endocrinology role includes clinical pharmacy management mentoring and training. It includes supervision ofrotational pharmacists through the medical units. The role will be responsible for supervising interns andstudents working in the medical units.
This role will be a clinical leader in medical pharmacy and work as an integral part of the extendedmedical team. This role will collaborate and work with other senior pharmacists in the department.
POSITION DUTIES
Provide high quality safe and cost-effective medical pharmacy practice
Collaborate with other senior pharmacists to run the day to day activities of the clinical pharmacist
team in the medical units and other areas as required.
Supervise other pharmacy staff involved in providing services including pharmacy technicianspharmacy interns and student pharmacists.
Ensure the timely and accurate supply of drugs by dispensing imprest maintenance and respondingto medication shortages.
Liaise with outside agencies to clarify relevant patient medication records to improve patient careand reduce risk.
Contribute to achieving the best use of medicines by enabling informed patient choice andimplementation of best practice
Ensure consistency in clinical pharmacy practices.
Develop expert knowledge and skills.
Act as the point of contact for medical pharmacy expertise for medical nursing and allied healthstaff.
Keep abreast of new developments in clinical pharmacy practice and implement new systems andprocesses that conform to best practice.
With the Senior clinical pharmacist Co-ordinate the training of Grade 2 pharmacists working in medical areas.
Participate in the oncall weekend and public holiday roster
Participate in the extended clinical pharmacy roster when requested by the Deputy Chief or ChiefPharmacist.
In collaboration with the Informatics pharmacist manage the automated dispensing cabinets inmedical areas.
Management supervision and training
Train rotational pharmacists to work in the general medical wards and other medical wards.
Oversee supervision of junior pharmacists intern pharmacists students and technicians working inmedical areas.
Identification of training and educational requirements and priorities for all clinical pharmacy staff.
Involvement in medical nursing and allied health education on relevant medical issues.
Participate in the pharmacy department education program including providing continuing
education sessions intern tutorials and journal club presentations.
Assist in the various university undergraduate and post graduate experiential placement programs.
Represent the Pharmacy Department at multidisciplinary meetings with the broader medical team
and collaborate with key staff across the organisation.
Assume responsibility and provide input as a member of the senior pharmacy leadership team.
Act as cover for the Senior Clinical pharmacist when requested by the Deputy Chief or ChiefPharmacist.
Together with the senior pharmacy leadership team develop and maintain policies procedures and
guidelines governing the scope of pharmacy services in medical areas.
Ensure there is ongoing review of all local clinical pharmacy procedures to ensure that these are
appropriate effective and followed by staff.
Ensure there is ongoing review of local medication policies.
Ensure clinical pharmacy staff compliance with pharmacy and hospital procedures relating to theirresponsibilities including but not limited to drug formulary drug guidelines and medication chartprescription and activity documentation.
Ensure clinical key performance indicators are recorded and deviation from target is reviewed and actioned. Research quality improvement quality assurance and quality use of medicines
Work in conjunction with the senior pharmacy leadership team on projects relevant to clinicalpharmacy.
Actively contribute to drug utilisation and medication evaluation.
Promote rational use of medicines by leading quality and safety projects.
Develop and contribute to pharmacy quality improvement projects.
INCUMBENT OBLIGATIONS

Ensure clinical care is undertaken within established procedures in order to provide safe clinical carefor patients/residents ensuring clinical risk is minimised
Ensure clinical staff in the local area work within their approved scope of practice and at all timeswith appropriate supervision.
Ensure all junior clinical staff are appropriately supervised and trained to provide safe clinical care
Promote a culture that supports learning and encourages reporting of errors
Implement systems to identify and manage risks and to deal with and learn from incidents andcomplaints
Implement all facets of the SVHM safety and clinical quality programme within clinical area managed
Collaborate with more senior levels of management to implement the SVHM safety and qualityagenda
Provide feedback to more senior management in relation to problems or issues that impact on safetyand clinical quality.
Ensure that consumers receive information in an appropriate and accessible format
Actively support consumers to make informed decisions about their treatment and ongoing care
Ensure consumers are aware of their rights responsibilities and how to provide feedback
Ensure all health and safety related policies procedures and directions are complied with in the areathey manage
Ensure all in the area they manage undertake annual Fire and Emergency Training and comply withfire and emergency procedures
Ensure all those in the area they manage treat others with respect behaving professionally and inaccordance with the SVHM Code of Conduct and undertaking annual Workplace Culture and EquityTraining
Conduct regular safety audits with Health and Safety Representatives and implement requiredimprovements.

SELECTION CRITERIA
ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS
Bachelor of Pharmacy or equivalent.
General Pharmacist Registration with AHPRA.

Broad range of experience (minimum of 5 years) in hospital pharmacy.
Extensive medical pharmacy experience.
