**Qualifications:**

- Board Certification

- REQUIRED

- Active Oregon License

- REQUIRED

- Drug Enforcement Administration (DEA) Registration and Controlled Substance Registration (CSR)

- REQUIRED - Basic Life Support (BLS) Certification

- REQUIRED - Local candidates are highly preferred.
