About the Role
- Assist clients and their whānau
- Collaborate with occupational therapists physiotherapists and social workers
- Understand interventions and rehabilitation plans to help older people and people with complex disabilities to regain independence and help them stay in their own homes for longer.
- The focus is on rehabilitation and working towards the patients goals whilst working closely with the families of the patients.

About You
- New Zealand Enrolled Nurse with a current annual practicing certificate.
- Nursing experience that includes working with stroke orthopaedic frail elderly and other neurological conditions.
- Demonstrate a strong understanding of optimising patient outcomes
- Facilitate in effective rehabilitation
- A patient centred and evidence-based approach
