Cornerstone Family Healthcare is actively recruiting for a Qualified Nutritionist to join our growing team in Port Jervis.
RATE OF PAY/SALARY: $32.00-$35.00 per hour (based on credentials)

JOB SUMMARY:
General Purpose: The ideal candidate possesses expertise that qualifies them to perform all nutrition-related duties at the WIC program including providing and overseeing high risk care participants.
This job currently offers two remote days per week after training is complete.
Description of Duties:
- Conduct a complete nutrition assessment determine nutrition risk and enroll participants.
- Conduct a breastfeeding assessment and provide participant centered counseling that helps a mother initiate breastfeeding establish a milk supply exclusively breastfeed and continue breastfeeding.
- Address common breastfeeding concerns and issue breast pumps.
- Document nutrition services provided including referrals and follow-up to referrals.
- Prescribe tailor authorize and reissue food package.
- Provide and document participant-centered nutrition education/counseling (individual/family sessions and facilitated group discussions)for participants
- Ensures referrals for lead testing and immunizations are performed.
- Track participants progress and document outcomes.
- Develop and implement high risk care plans and referral of high risk plans to the Registered Dietitians.
- Coordinates nutrition education including breastfeeding promotion and support that is responsive to the identified needs/interests of each high-risk participants
- Track high-risk participants progress in improving their health and document outcomes.
- Ensure the efficient flow of the assessment and education process.
- Provide leadership in the planning conducting and evaluation of Participant-Centered Nutrition Education services.
- Perform other related duties as assigned by the Site Supervisor and Program Director.
- Attend and participate in monthly department All Staff and other required meetings.
- Be familiar with and adhere to all clinical administrative and general policies and procedures as outlined in the Cornerstone Family HealthCare Policy and Procedure Manual and Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone Family HealthCare including but not limited to patient confidentiality financials and employee relations.

- Bachelors Degree in Nutritional Sciences community nutrition clinical dietetics foods and nutrition from a regionally accredited college or university or one recognized by New York State Education Department.
