Job Summary
The Community Services Navigator provides on demand resources to community members impacted by negative social determinates of health in the Coachella Valley. This access to programs and services will promote health and socio-economic wellness for the participants. The Community Services Navigator will ensure timely and coordinated access to appropriate health and support resources as well as services available in the Coachella Valley and in the region. The delivery of this service will be consistent with the policies and procedures of DAP Health and related program protocols.
Supervisory Responsibilities: None
Essential Duties/Responsibilities
- Develop a comprehensive resource list of services and programs within the Coachella Valley and the region for easy access from participants
- Comprehensive assessment of participants social determinants of health
- Assist eligible participants in understanding applications and forms during application processes for programs and benefits including Medicaid Cover California Cal Fresh Social Security etc.
- Act as a liaison between participants and community programs and services
- Act as an advocate and referral resource for participants
- Assist participants and services providers in problem solving when necessary and appropriate
- Maintain accurate records and appropriate documentation of all interaction with participants
Qualifications
Required Skills/Abilities
- Demonstrated familiarity with the Coachella Valley the Counties of Riverside and San Bernardino and community resources.
- Ability to communicate effectively with all levels of individuals both internally and externally.
- Appropriate written and verbal communication skills with all levels of individuals.
- Always establish and maintain professional boundaries with staff and community members
Education and Experience
- BA in social work psychology medical or social services or related fields or a minimum of 2 years of experience working with the public in a Social Services organization or program
- Bilingual Spanish preferred
Working Conditions/Physical Requirements
- This position is located at the Aloe Palm Canyon Apartments
- This job operates in an office setting and requires frequent times of sitting standing repetitive motion and talking
- Ability to lift up to 24 pounds
