Job Description Summary:

The critical care advanced practice provider supports the Intensivists in the coordination of care of adult patients in Intensivists service. Principal responsibilities include daily assessment monitoring medical management family education and medical care coordination in collaboration with the Intensivists. This care includes patient assessments through patient exams medical history interpretation of diagnostic procedures development and implementation of patient treatment plans education and follow-up using an organized multidisciplinary team approach to provide continuity of care throughout the continuum. During an episode of acute unstable and/or critical illness collaborates with Intensivists on plans and treatment to achieve and maintain physical system stability. Population focus for the Critical Care advanced practice provider is with patients with acute unstable and/or critical illnesses individuals with complex chronic illnesses who are experiencing episodic illness or an exacerbation of one or more chronic illnesses and their families with goal of treatment to restore the patient to previous or optimal level of physical function and health. The Critical Care advanced practice provider works within the scope specified by the applicable State Board as well as the Marin General Hospitals medical staff bylaws. The role supports Intensive Care Unit RNs in participating in Rapid Response Team (RRT) calls in the hospital setting.

Essential Functions and Responsibilities:

Critical Care Advance Practice Providers duties in the Intensive Care Unit include:

  • Conduct comprehensive medical and social history of individuals

  • Conduct comprehensive or focused physical examination as condition requires

  • Analyze data to determine health status

  • Order and interpret laboratory tests and other diagnostic tests for the purpose of diagnosis and/or screening

  • Make independent decisions regarding management and treatment of medical problems identified

  • Confer with Intensivist as needed throughout the shift on plan of care and status

  • Seek collaboration or referral to collaborating physician and other health care providers or agencies as needed

  • Formulate a plan of care with patient and/or family; family updates

  • Initiate appropriate non-pharmacologic interventions

  • Prescribe medication in accordance with state prescriptive guidelines

  • Assist with and perform procedures: arterial and central line insertion removal of chest tubes intra-aortic balloon pump and epicardial pacemaker wires removal of unneeded tracheostomy tube or reinsertion of tracheostomy tube only when well established tracheostomy present for greater than one month CXR and EKG assessment and plan of care based on findings

  • Rounding on all patients with primary RNs and Intensivist

  • Assess patients for possibility for transfer and writing transfer orders

  • Provide appropriate patient discharge planning and/or education

  • Provide information instruction and counseling on health maintenance health promotion social problems illness prevention illness management and medication use

  • Obtain informed consent from patients

  • New patients - initial assessment collaborate with Intensivist on plan of care and write admission and new orders as needed

  • Support Intensive Care Unit RN during RRT calls

  • Round on former ICU patients as appropriate in the Progressive & Metabolic Care Unit

  • Track outcome of interventions and provide additional intervention as necessary for optimum results

  • Assess cultural social and economic factors for each client and tailor care to those factors

  • Manage care of patients in a way that balances quality and cost

  • Represent the practice hospital and nurse practitioner profession at the state and federal level addressing bills concerning medical treatment and financial issues in healthcare

  • Participate in professional meetings committees seminars and other continuing education to keep abreast in professional practice to enhance professional growth to meet professional standards and maintain current certification

  • Any other responsibility that is deemed appropriate within the role of the Intensive Care Unit Advanced Practitioner

  • Teach groups of staff or clients about health-related topics

  • Participate in supporting the education/professional development of clinical staff

  • Administrative-Clerical Duties

  • Document findings and plan of care in medical record

  • Complete test requisitions

  • Complete referral and consultation requisitions

  • Coordinate care of patients.

  • Enhance communication between departments physicians hospitals and/or community agencies

  • Dictate transfer/discharge H&P interval/progress notes or follow-up summaries

  • Function under guidelines of HIPAA

Qualifications:

Prerequisite Skills:

  • Demonstrated excellent communication skills both oral and written are necessary.

  • Clinical research skills basic computer skills (access enter and retrieve data; Word and Excel skills preferred).

  • Must be able to work with autonomy adaptability flexibility and maintain composure during pressures and adverse conditions and work as an effective team member in a collaborative role with other disciplines.

Education:

  • Graduation from an accredited Physician Assistant or Nurse Practitioner Program.

License/Certifications:

  • Must be certified by the National Commission on Certification and licensed in the State of California

  • Must hold current national board certification by first day of work and maintain.

  • Must hold an active and current California Physician Assistant or Nurse Practitioner License

  • Must hold an active DEA license (or transferable to California) as required by the clinical specialty.

  • Must be eligible for payer enrollment with CMS Medicare and Medicaid.

  • BLS and ACLS within one year of hire.

  • Non-Physician Medical Staff Privileges with Marin General Hospital within one year of hire.

  • Must hold a valid Controlled Substance Registration Certificate from both the California Department of Public Health and the Drug Enforcement Administration

  • Must complete a minimum 30 hours of pharmacology continuing education every 6 years or as required to maintain prescriptive privilege.

  • Written practice guidelines will be maintained on site reviewed and updated annually by the PA and collaborating MD or medical director.

Experience:

  • Previous critical care experience preferred.

  • Knowledge and demonstrated competency in current Critical Care medical practice as it relates to Intensive Care Unit; knowledge of adult educational planning implementation and evaluation.

  • Demonstrated competency in clinical practice with a minimum of 1-3 years of experience.
