Key Responsibilities

• Deliver home visits in assigned territories (Phoenix & surrounding areas)

• Perform admissions evaluations recertifications and individualized care plans

• Administer medications wound care and treatments

• Monitor client health and report changes to supervisors

• Ensure accurate documentation via EMR and mobile devices

• Coordinate care and maintain strong communication with physicians and families

• Provide case management and follow-up support

Qualifications

• Active Registered Nurse (RN) license in Arizona

• Minimum 1 year of recent clinical experience (medical/surgical)

• Prior home health experience preferred (training provided if not)

• Graduate of an accredited nursing program

• Strong time management critical thinking and interpersonal skills

• Comfort with EMR and mobile documentation tools

• Reliable transportation to visit clients across the coverage area

Clinical & Technical Skills

• Proficiency in administering medications wound care and medical procedures

• Ability to conduct accurate patient assessments and implement care plans

• Strong understanding of infection control patient safety and best nursing practices

• Experience with electronic medical records (EMR) and mobile charting tools

• Knowledge of OASIS documentation (preferred but not required)

Professional Skills

• Excellent time management and organizational abilities

• Ability to work independently while following care protocols

• Strong critical thinking and problem-solving skills for patient care situations

• Solid case management and follow-up abilities
