The University of Miami Bascom Palmer Eye Institute has an exciting opportunity for a full time Social Worker LCSW in Miami Florida.
The Social Worker LCSW provides psychosocial assessments intervention and/or treatment and discharge planning to patients and families to assist them in coping with their hospitalization illness diagnosis and/or life situation and meet the continuing needs of patients post hospitalization. The Social Worker LCSW administers a broad variety of therapies and is the designated expert consultant or source of knowledge within the social work profession.
CORE JOB FUNCTIONS
Advocates for and helps clients gain access to resources while confronting their personal issues.
Counsels and provides crisis intervention for patients including assessment and treatment of behavioral problems.
Coaches patients on how to manage ongoing health conditions.
Provides therapy to help patients make positive behavior changes.
Assists patients and their families through individual group and education conferences to understand accept and follow treatment recommendations.
Refers patients and their families to specialized treatment programs in accordance with their needs and monitors and evaluates their progress.
Collaborates with the other departments to integrate care where needed.
Maintains current and appropriate documentation in patient chart.
Acts as a mandated reporter in suspected child abuse cases.
Adheres to University and unit-level policies and procedures and safeguards University assets.

CORE QUALIFICATIONS
Education:
Masters Degree in Social Work
Experience:
Minimum 1 year of experience
Bilingual: English/Spanish preferred
Certification and Licensing:
BLS Certified (within 3 months of hire)
Knowledge Skills and Attitudes:
Ability to communicate effectively in both oral and written form.
Ability to handle difficult and stressful situations with professional composure.
Ability to maintain effective interpersonal relationships.

Department Specific Functions
Able to provide systematic psychosocial screening and assessments regarding mental status and psychopathology and communicating pertinent information to the healthcare team.
On-call rotation for the facility to receive and facilitate after-hour calls.
Able to demonstrate the ability to advocate for psychosocial-spiritual concerns and ensure that care is safe efficient effective person-centered timely and culturally congruent.
Documents appropriately and thoroughly in the electronic medical record.
Formulate develop and implement a plan utilizing appropriate social work interventions.
Collaborate with the health care team and involve the patient/family in the development and implementation of plans.
Facilitate discharge planning across a multidisciplinary care continuum that includes interacting with consulting physicians third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.
Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient.
Be able to provide appropriate referrals depending on the needs of the patient.
Be able to assist with financial issues.
Facilitate relaxation exercises and assist patients and family members in achieving positive coping strategies.
Provide education and counseling to patients and families around issues related to adaptation to the patients illness and/or life situation.
May utilize crisis intervention brief and long-term individual group and family therapies.
Develop and facilitate support groups or special functions.
Address and facilitate advance directives and able to assist patient/family with completion.
Able to practice with commitment to provide evidence-based care.
Address survivorship issues
Provide education to other health care professionals.
Participates in or consults on research/education programs.
May function as a resource to other staff as appropriate.
Perform other duties as assigned.
