Hours: Part time Summary The Placement Coordinator is responsible for managing all aspects of individual and provider (caregiver) matching within the Shared Living Program and overseeing a Respite Program. This role includes recruiting and matching shared living and respite providers to the profiles of individuals seeking services ensuring successful placements that align with the needs preferences and goals of all parties. The Placement Coordinator develops and nurtures relationships with stakeholders and ensures program compliance with Massachusetts Department of Developmental Services (DDS) regulations. Essential Job Functions - Recruit interview and assess potential shared living and respite providers to determine eligibility and alignment with program standards.
- Conduct the required provider references and background checks in collaboration with the Human Resources department.
- Develop and maintain a provider database inclusive of provider applications screening status certifications and trainings and availability (for respite providers).
- Match individuals with providers based upon alignment of the assessed individual support needs with the interests and skills of the provider.
- For Shared Living placements
- Coordinate and review the on-site pre-placement review of the provider home to ensure site meets the needs of the referred individual and that all DDS OQE placement services requirements are met.
- Schedule and oversee introductory meetings and trial visits between participants and prospective providers.
- Collaborate with assigned Shared Living Coordinator to ensure successful intake and placement activities including orientation of the provider.
- For Respite Program matches
- Facilitate initial introductions and agreements between families and respite providers to promote successful placements.
- Coordinate schedules and placements for respite services ensuring all parties needs are met effectively.
- Review and provide training for families and providers regarding documentation and record keeping requirements.
- Monitor the quality of respite services address concerns and mediate challenges as necessary.
- Complete the review and execution of Shared Living provider agreements that outline the roles and responsibilities of the provider Shared Living Coordinator and Incompass. The agreement includes items such as background checks and requirements for the provider and anyone residing in the home insurance requirements home maintenance and preventive maintenance compliance requirements training mandates documentation and record keeping participation and compliance with home visits and subsequent findings as well as activities considerations and tasks/interventions to meet the unique support needs of the individuals being served.
- Review annual facility assessments for all Shared Living provider homes to ensure the physical site continues to meet all DDS licensing requirements and accessibility needs for the individuals.
- Work closely with the Shared Living coordinators and other team members to make progress with program goals (effectiveness) and improve processes (efficiency).
- Assist with the overlap of shared living caseload above the threshold until a new shared living coordinator is hired and provide coverage as needed to support placements during absence or vacancy of a Shared Living Coordinator.
- Supervises Respite Providers inclusive of managing work schedules approving timecards and coordinating training.
- Responsible for being familiar with and abiding by Agency Department and Program policies and procedures and for acting in a manner consistent with organizational values mission and goals.
- Act as a liaison between families and providers to ensure open communication and collaboration.
- Communicate effectively professionally and respectfully to build and maintain positive relationships with individuals families/guardians providers Incompass team members and other stakeholders to gather input and address any concerns as they arise.
- Attend staff meetings trainings and other relevant events/meetings as needed.
- Serve as a resource for shared living and respite providers offering ongoing training guidance and professional development opportunities.
- Conduct regular check-ins and evaluations to ensure placements remain successful and sustainable.
- Handle crisis situations according to agency policies and procedures seeking resources when necessary.
- Serves as a Mandated Reporter and is required to advise direct supervisor DDS and DPPC immediately if abuse and/or neglect of an Individual is suspected.
