Job Description
Location: Watertown NY 13601
Shift: Days (40 hours) rotating weekend coverage
Pay Rate: $28/hr - $37.10 (hr)
Responsibilities:
- Conduct comprehensive nutrition assessments for patients/residents
- Identify individuals at risk for malnutrition and implement early intervention strategies
- Perform Nutrition Focused Physical Exams (NFPE) to assess the degree of malnutrition
- Develop individualized diet plans and monitor their effectiveness over time
- Provide nutrition education to patients/residents families and healthcare staff
Qualifications:
- Bachelor of Science degree in Human Nutrition and Dietetics (required)
- Registered Dietitian (RD) or Registration Eligible (required)
- Certified Dietitian/Nutritionist (CDN) in New York State (preferred)
- Minimum of 1 year experience in acute care or long-term care setting (preferred)
- Proficient in PointClickCare Meditech MS Word Excel and Outlook
- Strong communication interpersonal and customer service skills
- Displays excellent customer service skills
