This W-2 hourly Psychiatric Mental Health Nurse Practitioner position includes responsibilities that span across the following three organizations all under shared leadership:
FOCUS #1 Community Wellness Outpatient Mental Health Center
FOCUS #2 Lighthouse Behavioral Health Center
FOCUS #3 Freedom Healthcare
You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies with hours responsibilities and compliance obligations tracked separately for each.
This role is structured as a 24-hour per week position with a portion of the the total weekly hours allocated to each organization. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care each employer will manage its own onboarding HR and compliance processes in accordance with applicable laws and organizational policies.

CLASSIFICATION:
This is a part-time W-2 employee position totaling 24 hours per week worked over three scheduled days with hours allocated across three affiliated companies as follows:

Collectively the five assigned professionals provide a total of 120 hours of leadership coverage per week which exceeds the COMAR requirements for each organization as follows:
This structure ensures robust clinical leadership and regulatory compliance across all entities with ample coverage to support oversight team collaboration and quality improvement initiatives.
WORK SCHEDULE:
The Psychiatric Mental Health Nurse Practitioner (PMHNP) will work every Monday and two other set weekdays each week from 8:00 AM to 5:00 PM on each assigned day. The schedule will include one full administrative day (Monday) and two days focused primarily on direct client care and clinical services.
COMPENSATION RANGE:
Compensation for this position ranges from $30.00 to $100.00 per hour and is commensurate with experience expertise verified credentials and available company budget.

ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel.
DUTIES AND RESPONSIBILITIES:
1. Facilitate and document coordination with external primary care providers as clinically appropriate and with proper consent.
2. Prescribe medications following a full face-to-face evaluation and assess for any contraindications prior to initiating a medication regimen.
3. Educate clients and when appropriate caregivers on the purpose effects and risks of prescribed medications.
4. Monitor and adjust medication regimens as clinically indicated; conduct evaluations at a minimum every 90 days for ongoing medication management.
5. Notify the clients primary care provider of new prescriptions or medication changes every six months or as clinically required.
6. Conduct peer reviews of other prescribers work and support documentation compliance across the clinical team.
7. Participate in interdisciplinary team meetings with HR Scheduling Patient Access and other departments to support clinical and administrative needs.
8. Conduct batch documentation audits including review and signature of referrals treatment plans biopsychosocial ASAMs and intern notes.
9. Collaborate with external agencies or treatment providers in cases involving shared clients or complex care needs.
10. Accept and review incoming lab results in ICANotes; use clinical judgment to adjust care plans accordingly.
11. Ensure all documentation is completed within expected timeframes using ICANotes and is coded appropriately using evaluation and management CPT codes.
12. Contribute to quality assurance audit readiness and performance improvement initiatives to ensure compliance with COMAR CARF Medicaid and internal standards.
13. Provide support and education to clinical staff related to medication protocols and service delivery expectations.
14. Submit periodic reports and summaries related to administrative and clinical oversight duties.
15. Administrative Leadership Monday (8:00 AM 5:00 PM)

PHYSICAL DEMANDS: Regularly walk stand or stoop; occasionally lift carry push pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle.
WORKING CONDITIONS: All work is performed in an office where staff is exposed to communicable diseases of patients.
COMPETENCIES AND SKILLS:

Candidates must meet the following minimum qualifications to be considered for this role:
Active licensure with the Maryland Board of Nursing (MBON) as a Certified Registered Nurse Practitioner (CRNP)
Masters Degree in Nursing from an accredited institution
Board certification as a Psychi9atric Mental Health Nurse Practitioner (PMH)
Possession of a valid DEA (Drug Enforcement Administration) registration
Possession of a valid CDS (Controlled Dangerous Substances) registration

The Psychiatric Mental Health Nurse Practitioner (PMHNP) serves as a critical clinical leader at Community Wellness Outpatient Mental Health Center (CWOMHC) providing direct psychiatric care medication management clinical oversight and regulatory leadership in compliance with COMAR 10.21.20.
In accordance with COMAR 10.21.20.10 this position may fulfill the responsibilities of both the Medical Director and Program Director when applicable. As Medical Director the PMHNP assumes responsibility for the therapeutic direction and medical quality of the program including diagnostic and treatment standards prescribing practices physician coverage and clinical supervision. As Program Director the PMHNP ensures administrative continuity regulatory compliance and multidisciplinary team oversight including during staff absences or vacancies.

The Psychiatric Mental Health Nurse Practitioner (PMHNP) at Lighthouse Behavioral Health Center serves a dual role as both a clinical provider and program leader within a licensed behavioral health and residential substance use disorder treatment setting. The PMHNP provides Medication Assisted Treatment (MAT) and psychiatric care ensures regulatory and quality compliance and may function in clinical leadership and administrative capacities in accordance with COMAR regulations.
Under COMAR 10.09.80.05 the PMHNP must hold DEA Schedule III authority and is authorized to bill for services using HHG 99203 HG 99204 HG 99205 HG 99211 HG 99212 HG 99213 HG and 99215 HG. The PMHNP delivers pharmacologic treatment using FDA-approved medications such as buprenorphine supports flexible tapering and withdrawal management and ensures that all services are clinically appropriate and client-centered.
The PMHNP also fulfills requirements for residential Level 3.1 (low intensity) Level 3.3 (medium intensity) and Level 3.5 (high intensity) providers by ensuring sufficient clinical coverage to conduct initial diagnostic work-ups manage medical emergencies and refer for identified medical or surgical needs. The PMHNP must be available for at least:
In addition the PMHNP serves as the Clinical Director as defined under COMAR 10.21.07.02(14) responsible for the therapeutic and rehabilitative direction of the program. This includes direct clinical supervision of counselors peer recovery staff and oversight of all care coordination with external providers.
The PMHNP may also serve as the Program Director and must be on-site at least 20 hours per week while fulfilling that role. The Program Director is responsible for the continuity of clinical operations regulatory compliance and staff supervision.
In the absence or vacancy of the Lighthouse Administrator the PMHNP will automatically assume the responsibilities of Acting Administrator ensuring uninterrupted program operations staff leadership adherence to internal policies and procedures and regulatory oversight per COMAR 10.47.01.06.
