City of Greeley: Work Live and Play in Greeley: SUMMARY:
Responsible for providing case-management support using the Housing First model Trauma Informed Care Harm Reduction and other evidenced based practice to individuals in need of housing and services. These include meeting someone currently homeless on the street encampments or in shelter and supporting them through every step of the housing process. From obtaining IDs applying for rental subsidy searching for apartments meeting with landlords and then providing intensive support once the person is housed. This may include and not limited to providing crisis intervention services developing goal plans facilitating referrals and serving as advocate on behalf of participant to assist them in securing services entitlements and support to reach their goals. The work is 80% based in the community making home visits and supporting people to become integrated back into their community.
The Outreach Case Manager positions are full-time term-limited positions funded through 2026 with the possibility of becoming permanent if additional funding is approved at a later date. This position is benefit eligible.

Ability to work independently in identifying and resolving issues concerns and problems.
Ability to acquire subject matter expertise in the functions and activities of the department including applicable laws rules regulations procedures and technical operations.
Ability to use troubleshooting and analytical skills; oversee corrections within assigned specialty; read interpret and apply complex technical documentation; analyze evaluate and integrate business processes and procedures.
Ability to write logical comprehensive concise reports and correspondence; demonstrate good customer service and project management skills; communicate effectively orally and in writing using language understandable to management and staff.

ESSENTIAL FUNCTIONS:
Conducts initial assessment to determine individual client needs (VISPDATs)
Builds positive relationships with a range of people including residents community leaders landlords local groups and organizations and local authorities
Participates in face-to-face and online activities trainings zoom meetings etc.
Attends meetings (huddles daily) CAHPS meetings home visits hotel checks community visits
Works in different environments (both office-based and on the ground) at times best suited to the community (including some evenings and weekends).
Completes DAP notes daily after having client interactions
Work collaboratively with community partners and other Departments/Staff in linking clients with needed services.
Performs other duties/special projects as assigned.
Assists clients in completing forms and applications to receive services related to health housing and employment
Assists clients in scheduling appointments
Coordinate transportation to assist clients attend medical appointments community appointments
Uses computer applications such as spreadsheets word processing calendar e-mail and database software in performing work assignments.
Provides crisis intervention by assisting clients access various social services and referring clients to alternate care facilities
Speaks before neighborhood groups and attends community meetings to establish communication and rapport; to promote services; and assist in resolving problems facing the community. Prepares and distributes informational literature.
May provide guidance and training over other staff members.

Work Environment:
Work is primarily conducted out in the community with exposure to various weather conditions and environmental factors uneven terrain and other risk factors.
Flexible schedule with requirements to work evenings weekends and holidays.
Frequent interactions with the public involving extensive talking or listening or crisis intervention to include verbal de-escalation and connection to community crisis resources.
Home hotel hospitals encampments bridges streets and other community settings.
Operates standard field equipment and tools requiring frequent or repetitive hand/arm movements.
Vision sufficient to interpret documents computer screens and assist the public in various outdoor settings; specific vision abilities include close vision distance vision color vision peripheral vision depth perception and the ability to adjust focus.
Manual dexterity is needed to accurately input retrieve and verify work assignments.
Frequent use of hands for handling reaching or feeling with occasional light to moderate physical effort including stooping kneeling crouching crawling and standing or walking for extended periods.
Frequent lifting of objects up to 20 pounds and occasional lifting of up to 50 pounds.
While most work is performed outdoors there may be occasional desk-based tasks or administrative duties (meetings trainings etc.).
