Job Summary
RN Nurse Case Manager (PRN)
Works in cooperation with the interdisciplinary team to effectively implement the Plan of Care. Provides nursing care in accordance to medical orders and the Plan of Care. Nursing care is provided for patients enrolled in the Hospice and Palliative Care Home Care Program.
Qualifications:
- Associate Degree or Bachelors Degree in Nursing. Bachelors degree preferred..
- A minimum of two years experience in a medical surgical environment home care or hospice program. Hospice and home care experience preferred.
- Ability to assess and respond to the complex needs of terminally ill patients and their families
- Skilled in nursing practice able to provide emotional support and acceptable of individual lifestyles
- Skilled in pain and symptom management
- Professional positive attitude required
- Ability to function as part of a team
- Ability to embrace the mission and culture of the organization
Duties:
- Manage the care of assigned patients by making visits to patients within the service area and by providing nursing care which includes patient/family teaching and support
- Communicates regarding the patients status with physician team members and other agencies involved in the care of the patient
- Attends team conferences staff meetings and in-services as assigned
- Documents patient/family contacts nursing care provided and other required documentation in an accurate and timely manner
- Adheres to all regulatory standards and agency policies as set forth
- Coordinates the transition of patients and changes in level of care
- Assists with referrals admissions and evaluation visits as needed
- Supports the overall culture of the organization
- Participates in organization sponsored events
- Performs other related duties as assigned
