MTM Pharmacist Help Patients Get the Most Out of Their Medications

Key Responsibilities
- Conduct comprehensive medication reviews (CMRs) with patients to identify medication-related problems.
- Provide targeted medication interventions (TMEs) and follow-up to support adherence and safety.
- Collaborate with prescribers health plans and care teams to resolve therapy issues.
- Document interventions clearly and maintain compliance with MTM program requirements.
- Educate patients on safe effective use of their medications and address barriers to adherence.
What Youll Bring
- Education: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree.
- Licensure: Active and unrestricted pharmacist license in the U.S.
- Experience: Retail hospital ambulatory care or prior MTM experience is a plusbut all licensed pharmacists are encouraged to apply including new grads.
- Skills: Strong communication patient counseling and ability to work independently.
