Empath Home Health a member of Empath Health is seeking a compassionate Home Health RN to provide skilled nursing care for patients in their homes throughout Hillsborough County (Tampa Lutz Odessa Carrollwood and surrounding areas). In this full-time role youll deliver individualized care build trusted patient relationships and help people manage their health with dignity and independence.

Schedule: MondayFriday with required weekend/holiday on-call rotations.

Locations: home health visits in Hillsborough County (Tampa Lutz Odessa Carrollwood and surrounding areas)

What Youll Do

Perform comprehensive patient assessments including OASIS assessments at required intervals
Develop initiate and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes self-care techniques and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless holistic care

Position Requirements

Graduate of an approved nursing program with an active Florida RN license
BSN or MSN preferred
Minimum of 1 year of direct patient care experience; prior home health RN experience preferred
Strong assessment critical thinking and patient education skills
Willingness to participate in weekend and holiday on-call rotations
